Save time by filling out our patient forms before your first appointment.

Please use the links below to fill out your new patient paperwork. This information will be transferred securely to our office for your first appointment. If you cannot load and complete the paperwork ahead of time, please be advised that you will need to fill it out electronically when you arrive for your first appointment.
When making a new patient appointment you will be asked to pay a $100 non-refundable deposit over the phone. This will secure your spot with us and serve as a credit to be applied towards the remaining balance due on the new patient appointment. Should you need to reschedule your appointment, the deposit will go towards the rescheduled visit as long as 24 business hours notice is given.

Cash, Check, Visa, Master Card, Discover, or American Express – Please note that if paying with a credit/debit card, there will be a service fee of 3.95% added to your total. This is a processing fee that does not go to the practice.ProHealth Center offers our patient form(s) online so they can be completed in the convenience of your own home or office. Download the necessary form(s), print it out and fill in the required information then fax us your printed and completed form(s) or bring it with you to your appointment.

  • What can I expect at my new patient appointment?

    The doctor and/or practitioner will meet with you to do a thorough consultation, health history review and exam, if needed. They will at that time determine what services are necessary for your health and devise an individualized program just for you!

  • What conditions can you help with at your office?

    We see patients with a variety of health concerns. Our protocols are written for each individual to fit their specific case and are very effective. We have helped patients with chronic fatigue, multiple auto immune disorders, diabetes, supportive cancer therapies, Lyme disease, metabolic disorders, thyroid concerns, and much more!

  • Do you offer imaging at your facility?

    Yes! We have digital x-ray equipment, which provides a more clear and accurate picture for the doctor. We also offer digital infrared technology, also known as thermography, scans. You can visit our thermography page to learn more by clicking this link.

  • Do I need a referral?

    No. You do not need a referral to be seen in our office. It’s a simple as calling our office at 864-681-0555 and one of our staff members will be happy to get you scheduled.

  • Do you see children at your facility?

    Yes. We can see children of any age at our facility, even newborns.

  • What should I do prior to my IV appointment?

    Please come to your appointment well hydrated and bring a light snack and water with you to ensure stable blood sugar levels during the duration of your IV appointment.
